The submission deadline 12 months from the date you attended the SAAD National Course.

It is possible to have requested an extention by submitting an extention request form.  If you have received an extention to you submsison deadline,  that is the date you should note as your submission deadline.

If you have missed the submssion deadline,  or know that you are likely to, please complete an Extention Request Form as soon as possible, and before submitting your portfolio.

Each of your supervisors,  who have supervised your logged cases will need to complete this section,  and must sign their name.

If any of your cases were supervised by someone for whom you had not submitted a Supervsior Nomination Form,  please complete a form as soon as possible, and before submitting your portfolio.

Every supervisor must be approved by SAAD to supervise your cases on the SAAD scheme

1. One form should be completed for each supervised case that was completed after you have passed your assessment and returned your confirmation  statement.
2. They should be completed in date order, to reflect your progress.
3. Remember to indicate if you proivded the sedation only, or the sedation and treament. At least 10 of your supervised cases shoudld be where you have provided the sedation and the treatment.
4. The date should be in the format dd.mm.yyyy
5. The name of the drug used should be entered in full.
6. The dosage should be recorded in mg. If it is recored in ml,  then the concnetration used should also be recorded.
7. Refer to page 3 for the completion of 'ASA' and 'Comments' sections.
8. On consultation with your supervisor, each of the areas should be scored to reflect your management of the case. If you  were not involved in an area it shodul be graded as U/C.
9. Your trainer should complete the sections 'What went well?' and 'What could be improved?' These comments are important for your professional development.

10.  Likewise, you should complete the section 'Trainee to record any reflections on this case'.

11. Your supervisor should both sign and print their name on each case. The person signing should have supervised the case,  and should be one of you supervisors listed on page 2.

1. One checklist should be completed for each location where supervised cases were logged. Additional copies can be downloaded.

3. Each field should be read and completed accurately, with notes added where further explanation is required. 

 Once you have completed all sections of your portfolio this page should completed by you and your supervisor.
