Title: Remimazolam delivered via infusion pump for intravenous sedation in dentistry: a case series

Abstract:
Intravenous sedation plays an important role in dentistry, where patients may present with significant anxiety, behavioural challenges, or complex medical needs. Midazolam has traditionally been used for dental sedation; however, its intermittent administration can be associated with variable sedation depth, limited titratability, and unpredictable recovery, particularly during prolonged procedures. Remimazolam is a short-acting benzodiazepine with rapid onset, predictable titration and favourable recovery characteristics. When delivered via an infusion pump, it allows continuous administration of the sedative drug. 

This paper presents a case series of three patients treated under intravenous sedation with remimazolam delivered via infusion pump in a dental setting. The cases were selected to illustrate key clinical scenarios: prolonged sedation, comparison between manual dosing and infusion delivery, and the use of remimazolam across different sedation models, including both single operator-sedationist and separate sedationist approaches. Across these cases, infusion-based remimazolam provided stable and consistent sedation with predictable recovery profiles. The use of an infusion pump enabled continuous drug delivery and maintenance of a steady sedation level throughout treatment.

This case series illustrates the potential practical advantages of infusion-based remimazolam sedation and contributes to the emerging evidence base supporting its use in dentistry.
